Clarification Statement/Worksheet Federal regulations require financial aid staff to review and correct all conflicting information indicated on the Student Aid Report (SAR). Supplemental Loan Request The financial aid office will review your supplemental loan request and determine your eligibility. Before completing this form be sure to consider your future loan debt. Ability-to-Benefit (ATB) Testing Ability-to-benefit testing is required for students who apply for financial aid and do not have proof of either a standard high school diploma or GED on file.

Bank of America Personal Signature Card Form CashPay is a VISA®-branded card, which allows the College to offer direct deposit to students. The student has access to their financial aid disbursement 24 hours a day, seven days a week.

Child Care Grant Application Please return this form with the required documents to your campus enrollment services office. Incomplete applications will not be processed.

Electronic Funds Transfer (EFT) Enrollment Form EFT Enrollment Form is a required document for all FCCJ funded grants and scholarships. EFT authorizes funds to be deposited directly into your bank account. You may use the enrollment form to provide your current account information to sign up for a free Bank of America Campus Edge checking account.

IRS Withholding Documentation The IRS Withholding Document is required for students who are not U.S. citizens or Permanent Residents that are receiving financial aid, scholarships or grants.

Scholarship FERPA Form This form authorizes FCCJ to send pertinent information to donors for scholarship recipients. Please see your application requirements in Connections (Artemis) to determine if you need to complete this form for your scholarship award.

Student Aid Authorization The Student Aid Authorization Form gives FCCJ permission to deduct miscellaneous fees and prior debts from financial aid funds.

Student Aid Appeal Form Students whose aid is cancelled may appeal this decision ONLY if there are extenuating circumstances (medical or personal problems). To appeal, the student must complete and return the Student Aid Appeal Form to the home campus enrollment services/financial aid office. All supporting documentation must be attached or the appeal form will not be accepted.

Commonly Requested Documents/Documentation

• Athletic Aid Form: This form is submitted by the athletic department on behalf of the student.
• Birth Certificates: The financial aid office may request a copy of your or your dependent’s birth certificate.
• Free Application for Federal Student Aid (FAFSA): Apply online and FCCJ will receive your information electronically.
• Honors Contract: Students awarded an honors scholarships must return a signed Honors Contract to the scholarship department.
• Income Tax Return and W-2s: If you did not keep a copy of your tax information contact the IRS at 1.800.829.1040. A description of the tax year required can be found in Connections (Artemis) on your financial aid checklist. Dependent students must submit a copy of all tax information required on the FAFSA both student and parents. Independent students must submit a copy of all tax information required on the FAFSA both student and spouse (if married).
• Items Related to Proof of Financial Aid Independent Status: Students who are under the age of 24 may be required to submit proof of their independent status. Examples of independent proof are marriage certificate, child’s birth certificate, DD214 (U.S. military standard separation document), etc.
• Items Related to Immigration Status: If your FAFSA did not match with the Department of Homeland Security, you may be required to submit additional immigration documents. Examples, passport, resident alien card, etc.
• National Letter of Intent: This form is submitted by the athletic department on behalf of the student.
• Scholarship Letter of Acceptance: This letter is mailed with certain scholarship award packages. Please return your Letter of Acceptance to the Scholarship department.
• Selective Service: If you are required to provide proof that you registered with Selective Service, visit the Selective Service System Web site.
• Social Security Benefits Statement: To request a Replacement SSA-1099, visit the Social Security Web site.
• Social Security Card: The financial aid office may require a copy of your or your parent’s social security card.
• Talent Grant Agreement: This agreement is submitted by the talent grant administrator on behalf of the student.
